Hannah Aba BSN, RN, CDP - AGENCY SUPERVISOR
Hannah brings a unique perspective that comes from years of hands on experience in most aspects of the home health and healthcare industry while serving in several key positions including Administrator, Director of Nursing, as well as assisting in operational and managerial duties.
For several years she has lead teams of nurses to successful marks in quality assurance and positive patient outcomes by consistently ranking top in Home Care Elite. Most recently, spearheading the team that has received not one but TWO clinical deficiency free survey outcomes from the Illinois Department of Health (IDPH). With 15 years of experience it has molded her into a force that is well suited to passionately serve the increasing number of seniors in our population. She says “With awareness and deep appreciation of what it takes for our clients to be provided with the highest quality of skilled care our team not only aims to help our patients transition and live as independently as possible at home. We aim to give our patients that thoughtful and humane approach to lessen the stress and burden when both the patients and caregivers are most in need.”

Clara Jayne Guillermo is a dedicated Home Health Administrator bringing a commitment to excellence in healthcare administration. With a background in Bachelor of Science in Nursing, she is poised to contribute dynamic leadership to Humanity Home Health. Passionate about providing quality home health services, Clara Jayne is focused on ensuring compassionate care and operational efficiency.

Kate Fruhstuck - Director of Business Development
Kaitlin strives to bring her utmost knowledge from experiences brought together to mold into one. Those being bedside patient care as a Certified Nursing Assistant (CNA), Health and Wellness Coaching (CPT), and her Patient Support Coordinating skills stemming from her time in Customer Service. Her diverse experiences bringing her into the role as the Director of Business Development here at Humanity Home Health have given her the greatest perspective to yield results in encouragement, support, teamwork, and growth in Humanity as well as the building of relationships with those we are privileged to work alongside of in providing the best care possible for our communities. Growing up surrounded by the greatness of healthcare built the love and passion within her to be a helping hand and the light in one’s journey to healing and health. Her mission in not only this role, but life is to protect every person that she is granted the ability to cross paths with and ensure their feeling of safety and comfort in her presence. The drive she has for doing great things, and making an impact in our healthcare system is what fuels the difference that the community will continue to see and feel as we grow and develop here at Humanity!
