Care provided by a qualified professional, through a loving home care agency is an ideal option for many families and can bring back the joy in family life. You can make a referral for yourself or family member, as may Hospital social workers, case managers, discharge planners and community based organizations. However, insurance requires a physician order for certified home care. Humanity Home Health works with these parties to follow up on the physician order to acquire the authorization of a treatment plan, known as a Plan of Care.
Covered services include:
- Part-time or intermittent skilled nursing care
Services may include injections, or teaching self-injection; tube feedings; catheter changes; observation and clinical assessment of condition; manage & evaluate Care Plan and wound care.
- Skilled Therapy Services
- Physical Therapy including gait training and exercises to regain and strengthen weakened body areas.
- Occupational Therapy helps regain the ability to do usual Activities of Daily Living (ADL) such as eating and dressing
- Speech and Language Pathology services include exercises to regain and strengthen speech and language skills
- Medical Social Services
When ordered by your physician, we can help with social and emotional concerns related to your illness or condition. Humanity Home Health can also assist in accessing counseling or finding community based resources.
- Home Health Aide
When combined with skilled nursing or therapy, a home health aide can provide personal care services such as bathing; toileting and dressing.
Humanity Home Health is a valuable resource in gaining medical supplies such as wound dressings and catheters. We also collaborate with providers of Durable Medical Equipment (DME) such as a wheelchair or walker
